Betacap is a prescription-only medicine (POM) and cannot be purchased over the counter. Your online consultation with Access Doctor will determine whether Betacap is right for your scalp condition. You will need to disclose any other medications you're taking (especially other steroid treatments), known allergies, skin infections, broken scalp skin, pregnancy or breastfeeding status, and any previous adverse reactions to steroid treatments.
Do not use Betacap for more than 4 weeks without medical review. If you have seen no improvement after 4 weeks, stop and contact your doctor. This information is for educational purposes and does not replace professional medical advice or the patient information leaflet inside the pack.
Betacap is a prescription-only, clear gel-like liquid used to treat two common inflammatory scalp conditions: scalp psoriasis (thick, red, scaly patches) and seborrhoeic dermatitis (an itchy, scaly, red rash on the scalp). It contains the active ingredient betamethasone valerate 0.1%, a potent corticosteroid that calms inflammation and reduces redness, itching, and flaking. Betacap is reserved for cases that haven't responded well to weaker treatments (milder steroids or medicated shampoos). The 100ml bottle has a special nozzle for applying the liquid directly to the scalp through the hair. It's important to note that Betacap is not a shampoo—it's a leave-on treatment you apply and leave on, not a product you lather up and rinse out.
Betamethasone valerate works by entering the cells in your scalp and calming down the overactive immune response that causes psoriasis and seborrhoeic dermatitis. It stops the production of the inflammatory chemicals that make your skin red, swollen, and itchy. It's a potent steroid, which means it's significantly stronger than over-the-counter hydrocortisone treatments. NHS formularies across the UK recommend Betacap as a first-line treatment for moderate scalp psoriasis. Betacap also contains a coconut oil derivative that helps moisturise the scalp and makes the solution easier to apply to dry, scaly skin.
The "ABC" method (Apply sparingly, Betacap left to dry, Continue as directed) from the manufacturer Dermal Laboratories makes the process easy to follow. Here is the step-by-step routine: 1. Preparation: Make sure your hair and scalp are completely dry before you start. If you have just washed your hair, wait for it to dry fully. 2. Part Your Hair: Use your fingers or a comb to part your hair and expose a small area of affected scalp. 3. Apply the Liquid: Using the special nozzle, gently squeeze a few drops onto the affected area. Do not apply large amounts at once—the product spreads easily. Try to get it onto the scalp, not just the hair. 4. Gently Rub In: Gently massage the liquid into the scalp using your fingertips. 5. Let It Dry Naturally: Do not cover your head, use a hairdryer, or sit near a fire or flame. Betacap is highly flammable until dry. It dries quickly, leaving your hair slightly stiff but not greasy. 6. Wash Your Hands: Wash your hands thoroughly after applying to avoid accidentally spreading the steroid to your face or eyes. 7. Leave It In: Do not wash your hair for at least eight hours, or ideally overnight. You can apply it at night and wash it off in the morning with a regular or medicated shampoo.
There are two phases to treatment: the intensive phase and the maintenance phase. Intensive Phase (starting treatment): Apply Betacap twice a day (morning and night) to the affected areas. Continue this twice-daily application until you notice a clear improvement—usually within a few days to a couple of weeks. Maintenance Phase (keeping it under control): Once you have improved, you can reduce your use to once a day, or even just a few times a week to keep the condition under control. The goal is always to use the lowest possible dose needed to keep your scalp comfortable. Maximum Duration: - Scalp Psoriasis: Up to 4 weeks without review, then step down - Seborrhoeic Dermatitis in Children: No more than 5-7 days total - Seborrhoeic Dermatitis in Adults: Usually responds quickly—reduce once improved
If you use Betacap twice daily for four weeks and see no improvement at all, you should stop and speak to your doctor. They may need to check the diagnosis, consider a different treatment option, or use a different class of medicine, such as a vitamin D analogue or a very potent steroid like clobetasol propionate.
Yes, Betacap washes out with normal shampooing. You can use a regular shampoo on the days you wash your hair. You should, however, avoid washing your hair immediately after applying Betacap, as this will wash the medicine off before it has time to work. The best routine is to apply Betacap at night, leave it in for a minimum of 8 hours, and then wash your hair the next morning with a regular or medicated shampoo. Some NHS formularies even recommend using Capasal Therapeutic Shampoo once or twice a week alongside Betacap to help remove heavy scales and keep the scalp clean.
Yes, but with strict limits and only under medical supervision. Betacap is licensed for use in children over the age of one year. Important paediatric rules: - For seborrhoeic dermatitis in children, do not use for longer than 5-7 days in total. - For scalp psoriasis in children, use only under close medical supervision and for the shortest possible time. - Do not use in children under one year of age. - Long-term continuous use in children should be avoided because their larger skin-to-body-weight ratio means they absorb more steroid, which can lead to adrenal suppression. Monitor your child for signs of steroid absorption: unusual tiredness, irritability, poor sleep, not gaining weight as expected, or getting very ill from a minor cold. If you see any of these, stop the treatment and contact your doctor urgently.
The official data sheets state that there is inadequate evidence of safety in human pregnancy, and animal studies have shown a very small risk of developmental abnormalities. However, the amount of steroid absorbed from the scalp is generally low. Many dermatologists would still use a short course of a potent steroid like Betacap on a limited area if the psoriasis is severe, because the benefit of controlling the disease outweighs the theoretical risk. Pregnancy: Use only if your doctor decides it is clearly necessary. Do not start using it without discussing it with your prescriber if you are pregnant or planning to become pregnant. Breastfeeding: If you use Betacap, ensure it is completely dry before nursing or holding a baby against your chest. Do not apply it to the scalp immediately before skin-to-skin contact.
Most people tolerate Betacap well, especially when used for short courses. However, side effects can occur. Common (affect less than 1 in 10 users): - Mild stinging or burning upon application - Itching or dry skin at the application site Less common but more significant (usually from overuse): - Thinning of the scalp skin (atrophy) – this can be permanent, so if you notice your scalp skin looks shiny, fragile, or you can see veins (telangiectasia), stop and see your doctor. - Allergic contact dermatitis – the skin becomes more red, itchy, and swollen beyond the original psoriasis plaques. - Acne or small bumps on the scalp or forehead - Unwanted hair growth in the treated area Rare side effects (seek medical attention): - Pustular psoriasis: In rare cases, using potent steroids on psoriasis can trigger a serious form called generalised pustular psoriasis, where the skin develops pus-filled blisters. If you see new blisters, stop Betacap immediately and call your doctor. - Adrenal suppression: Signs include unusual fatigue, dizziness when standing up, nausea, weight loss, and poor recovery from minor illnesses. - Eye problems: If you accidentally get Betacap in your eyes, it can, in rare cases, cause raised eye pressure (glaucoma) or cataracts. Symptoms include blurred vision, eye pain, or seeing halos around lights.
Topical steroid withdrawal (also known as red skin syndrome) is a recognised phenomenon where, after prolonged use of a potent or very potent steroid, stopping the treatment causes a severe rebound rash. The skin becomes intensely red, burns, stings, itches, and may ooze—often much worse than the original condition. Should you be worried? No—if you use Betacap as directed. TSW is associated with months or years of continuous use, often on delicate areas like the face or armpits, or using large amounts over large body areas. If you have been using Betacap for more than 12 weeks continuously on a large area of your scalp, do not stop abruptly. Instead, speak to a doctor about a tapering plan (e.g., reducing from twice daily to once daily, then to every other day, before stopping). For a standard 4-week course of treatment, TSW is not a realistic concern.
Do not use Betacap on: - Active infections on the scalp: bacterial (e.g., impetigo), fungal (e.g., ringworm), or viral (e.g., cold sores, shingles) - If you are allergic to betamethasone valerate or any of the other ingredients (e.g., coconut oil derivative, isopropyl alcohol) - On children under one year of age - On the face, armpits, or groin – Betacap is designed specifically for the scalp. The skin on your face and other areas is much thinner and more prone to thinning and other side effects. Do not use Betacap for alopecia (hair loss) either—it is not effective for that condition.
Betacap itself does not directly cause permanent hair loss. However, severe scalp psoriasis often comes with temporary hair shedding—the thick, inflamed plaques can grip onto several hair follicles, and when the scale comes off, clumps of hair can come with it. This shedding is from the inflammation, not the steroid. Once the psoriasis is treated and the scalp calms down, hair typically grows back normally. If you are noticing significant hair loss, speak to your doctor.
Storage: - Store below 25°C in a dry place. - Protect from light. Keep the bottle in its outer carton between uses. - Keep it out of the sight and reach of children. - Do not freeze or store in a hot bathroom cabinet. Shelf life: The shelf life is 3 years from the date of manufacture. Once opened, the product does not have a specific "use within" period, but as a good rule, if you haven't used it for several months, check that it still looks clear and does not have any particles or an unusual smell before using it again. Disposal: Do not throw Betacap away in household waste or pour it down the sink or toilet. Take any leftover or expired medicine to your local pharmacy, where they will dispose of it safely for you.
