Mirvaso Gel is a prescription‑only topical gel that contains the active ingredient brimonidine (specifically brimonidine tartrate). It is used specifically for the symptomatic treatment of facial redness (erythema) caused by rosacea in adults. Importantly, Mirvaso is not a treatment for the spots and pimples of rosacea. It is designed to treat the persistent redness that is a hallmark symptom of this skin condition, which can be a source of embarrassment and frustration for many people. It is not used to treat acne or any other skin condition.
To understand how Mirvaso works, you must first understand what causes the redness in rosacea. In rosacea, the small blood vessels in your face are abnormally dilated, meaning they are wider open than they should be. This allows more blood to flow close to the surface of your skin, which is what you see as persistent redness or flushing. Brimonidine is a highly selective alpha-2 adrenergic receptor agonist. In simple terms, it is a powerful vasoconstrictor, which means it works by causing these dilated blood vessels on your face to narrow or constrict. When you apply Mirvaso Gel, it acts directly on the receptors in the blood vessel walls, causing them to tighten and reduce the amount of blood at the skin's surface. This results in a noticeable and often dramatic reduction in facial redness.
Mirvaso has a very specific application method, and it is crucial to follow it correctly for both safety and effectiveness. You only apply it once every 24 hours. It does not matter what time of day you apply it, as long as you are consistent. The effect of one application lasts for up to 12 hours. Here is the correct, step-by-step method: Start with a clean, dry face. Gently wash your face with a mild, non-irritating cleanser and pat it completely dry. Use a 'pea-sized' amount. The maximum recommended dose for one application is a total of five pea-sized amounts of gel. One small blob of gel, about the size of a garden pea, is applied to each of the five areas of your face: your forehead, your chin, your nose, and each cheek. Spread it evenly. Using your fingertips, apply the gel smoothly and evenly as a thin layer across your entire face, not just on the reddest patches. Avoid sensitive areas. Do not get the gel in your eyes, on your eyelids, on your lips, or inside your mouth or nose. Do not apply it to any irritated or broken skin, such as cuts, scrapes, or sunburn. Wash your hands. Immediately after applying, wash your hands thoroughly with soap and water to remove any residue. Wait for it to dry. If you want to use other skincare products, such as a moisturiser, or apply makeup, you must wait until the Mirvaso gel has completely dried on your face.
You should see the gel starting to work quite quickly. In clinical studies, patients began to see an effect within 30 minutes of their first application. The redness-reducing effect of a single dose of Mirvaso lasts for up to 12 hours. This means you could apply it in the morning before work or a social event, and your facial redness should be significantly reduced for most of the day.
You can use Mirvaso once a day for as long as you have facial redness from rosacea that you want to control. It is a long-term treatment for a chronic condition, not a short course of therapy like an antibiotic. However, it is very important to know that rosacea symptoms, including the redness, will return after the effect of the gel wears off. Mirvaso is a symptomatic treatment that manages the redness on a day‑to‑day basis; it does not cure the underlying condition.
The most significant and well-documented side effect of Mirvaso is the exacerbation of rosacea symptoms, often referred to as 'rebound redness'. This can happen either a few hours after the gel wears off (approximately 8–12 hours after application) or, in some cases, very soon after the gel is applied. When rebound occurs, your rosacea symptoms, particularly the redness and flushing, can return and may be worse than they were before you applied the gel. This can be a very distressing experience. In clinical studies, this symptom exacerbation occurred in up to 16% of patients using Mirvaso. It is important to understand that this is a known and relatively common risk, not just a rare possibility. To help manage the risk of this rebound effect, you must follow a specific starting protocol. Do not just start using the full dose. You should initiate treatment with a smaller amount of gel (less than the five pea-sized amounts) for at least the first week. You can then gradually increase the amount you use, based on how well you tolerate it and how your skin responds. This gradual introduction helps your skin get used to the medication and allows you to find the smallest effective dose that controls your redness without triggering a rebound.
Beyond the risk of rebound redness, other side effects can occur, although they are less common. Very common side effects: In clinical studies, symptom exacerbation (the rebound redness) was 'very common', meaning it occurred in more than 1 in 10 users. Common side effects: Other side effects that occurred in up to 1 in 10 users include redness, flushing, a skin burning sensation, and contact dermatitis (a type of skin rash). Uncommon side effects: Systemic effects like dizziness have been reported, affecting less than 1 in 100 users. More serious systemic effects like hypotension (low blood pressure) and bradycardia (a slow heart rate) are rare, affecting less than 1 in 1,000 users.
There are several situations where you should avoid using Mirvaso. Do NOT use Mirvaso if: You have damaged or irritated skin on your face. This includes sunburn, rashes, cuts, or if you have recently had a chemical peel or laser therapy. Applying the gel to damaged skin can lead to increased absorption into your body and a higher risk of systemic side effects. You are taking monoamine oxidase inhibitors (MAOIs) for depression (such as selegiline or moclobemide) or certain tricyclic or tetracyclic antidepressants. These medicines can interact with Mirvaso. You have ever had an allergic reaction to brimonidine or any of the other ingredients in the gel. Use with EXTREME caution (and tell your doctor) if you have: Severe or unstable cardiovascular disease, including problems with blood pressure. Raynaud's phenomenon, depression, cerebral or coronary insufficiency, orthostatic hypotension, scleroderma, or Sjögren's syndrome. In these conditions, the blood vessel-narrowing effect of Mirvaso could potentially be harmful. You are pregnant or breastfeeding (see below).
Yes, you can. Mirvaso is designed to be used alongside other topical treatments for the inflammatory lesions (the spots and bumps) of rosacea. For example, your doctor might prescribe Rozex Gel or Finacea Gel to be used in the morning and Mirvaso later in the day, or vice versa. The important rule is that you should not apply the other product immediately before applying Mirvaso. You should apply one, allow it to dry completely, and then apply the other. Always follow the specific advice given by your doctor or the ordering clinician.
There are no well-controlled studies of Mirvaso in pregnant women. It is known that brimonidine can cross the placenta in animal studies, but the relevance to humans is not fully known. Because of this lack of safety data, Mirvaso should only be used during pregnancy if your doctor considers it clearly necessary. It is also not known if brimonidine passes into breast milk or if it could harm a nursing baby. As a precaution, you should not breastfeed while using Mirvaso.

Store Mirvaso at room temperature, below 25°C. Keep it in a dry place away from direct sunlight and heat. Keep the tube tightly closed when not in use. As with all medicines, keep it out of the sight and reach of children. Do not use the gel after the expiry date shown on the tube. Any leftover or expired medicine should be taken to your local pharmacy for safe disposal.
