How Mounjaro dosing works
Mounjaro (tirzepatide) is a once-weekly injection, taken on the same day each week, with or without food, at any time of day. It comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg — each supplied as its own pre-filled KwikPen.
Nobody starts at the top. Everyone begins on 2.5mg, and the dose rises in 2.5mg steps, with a minimum of 4 weeks on each dose before any increase. There's a good reason for the slow climb: Mounjaro's most common side effects — nausea, and other digestive symptoms — are strongly linked to how quickly the dose rises. Titrating slowly gives your gut time to adapt at every stage.

Here's the standard titration pathway for weight management, assuming each dose is tolerated and an increase is clinically appropriate. Your prescriber may hold any step for longer — the weeks shown are minimums, not a race.

Never stack pens to make a dose. Each pen is a single fixed strength — taking two 2.5mg doses is not the same as a prescribed 5mg step and isn't safe or licensed. Only ever inject the exact dose on your prescription, once a week.
Finding your maintenance dose
A common misunderstanding is that everyone is meant to end up on 15mg. They aren't. For weight management, the recommended maintenance doses are 5mg, 10mg and 15mg — the 7.5mg and 12.5mg strengths are stepping stones between them.
Higher doses did produce more weight loss on average in the SURMOUNT-1 trial, so there's a real dose-response relationship. But averages aren't individuals: plenty of people lose steadily at 5mg or 10mg with fewer side effects than they'd have higher up. The right maintenance dose is the lowest one that keeps your weight moving in the right direction with side effects you barely notice.

Missed doses and changing your injection day
Life happens — holidays, night shifts, a pen left in the wrong fridge. The rules here are simple:

Check how late you are
Count from your usual injection time. The cut-off that matters is 4 days (96 hours).

Within 4 days: take it
Inject the missed dose as soon as you remember, then carry on with your normal weekly day.

Past 4 days: skip it
Miss that week's dose entirely and take your next one on the usual day. Don't try to catch up.

Changing your injection day
You can switch to a new weekly day whenever you like, as long as at least 3 days (72 hours) separate two doses.
Never inject a double dose to make up for a missed one. Taking doses too close together sharply increases the risk of severe nausea, vomiting and dehydration. If you think you've taken too much Mounjaro, contact your prescriber or call NHS 111 for advice.
Managing dose increases
Most side effects cluster around two moments: your very first injections, and the days after each dose increase. That's expected — your body is adjusting to a higher level of tirzepatide — and for most people the symptoms fade within days.
If a new dose is hitting you harder than expected, tell your prescriber rather than pushing through. The schedule has built-in flexibility: you can stay longer at your current dose, or step back down and try again later. Nothing about the licence forces you upwards on a timetable.
