How to Apply Betnovate Scalp Application: Technique, Timing and Safety

When to get urgent help: Call 999 or go to A&E if your skin quickly turns red and you develop white, pus-filled bumps that join together – you may also have a fast heartbeat, a high temperature, or feel hot, cold or shivery. These can be signs of a more serious condition, such as pustular psoriasis, or an infection. Also call 999 if you have swelling of the face, lips, tongue or throat, or difficulty breathing. Stop using Betnovate and contact your prescriber, GP or NHS 111 if your scalp condition gets worse, the skin becomes swollen, you develop a widespread rash, or redness and burning spread beyond the treated area after you stop.
Key fact: Technique matters as much as the medicine. Betnovate Scalp Application (betamethasone valerate, equivalent to 0.1% betamethasone, a potent corticosteroid) is applied sparingly to a dry scalp, on affected areas only, and left to dry naturally — it is a leave-on treatment, not a wash-out one.

- Confirmed diagnosis — Betnovate Scalp Application is licensed (SmPC) for scalp psoriasis, seborrhoeic dermatitis of the scalp and inflammation associated with severe dandruff. It must not go on infected skin.
- Dry hair and scalp — if you have just washed your hair, dry it before applying (without a hairdryer). Water dilutes the liquid and spreads it beyond the areas that need it.
Step-by-step application technique

Wash and dry your hands
Clean hands before you start; you will wash them again afterwards.

Part the hair over an affected area
Work section by section, exposing the scalp where the redness or scaling is. Good lighting and a mirror help for areas you cannot see.

Apply a small amount to the scalp
Use the bottle’s nozzle to apply the liquid directly to the affected skin — not the hair. A thin film is enough; more is not better.

Rub in gently if you wish
The leaflet says you can rub the liquid in, but you don’t have to. If you do, use a fingertip so it stays on the affected patch.

Repeat across each affected area
Re-part the hair and work across the scalp until every affected patch is treated — and only the affected patches.

Let it dry naturally
Do not rinse it out, cover the scalp, or dry your hair with a hairdryer.

Wash your hands thoroughly
Remove all liquid from your hands and avoid touching your eyes. If you apply it for someone else, wash your hands afterwards or wear disposable gloves.

Do’s and don’ts
Do: affected areas only
There is no benefit to treating healthy scalp — it only increases steroid absorption.
Do: keep a routine
Applying at consistent times makes the course easy to complete without doubling up.
Don’t: get it in your eyes
Be very careful not to get the liquid in your eyes, and do not touch your eyes until you have washed your hands.
Don’t: continue unreviewed
Potent steroids are short-course treatments. Have your treatment reviewed at 4 weeks.
Flammability: the safety rule people miss
Betnovate Scalp Application is flammable. Keep it away from open fires, flames and all sources of ignition, including smoking, while you apply it and immediately afterwards. Store the bottle tightly closed, away from heat, below 25°C.
This is the most commonly overlooked instruction with alcohol-based scalp treatments. The risk is greatest while the liquid is wet, and it includes cigarettes, gas hobs and other heat sources. The leaflet also advises not to dry your hair with a hairdryer.
Using it alongside other scalp treatments
Betnovate Scalp Application fits into combination plans in a set order:
- With ketoconazole shampoo — for seborrhoeic dermatitis: wash with the shampoo on shampoo days, dry the hair, then apply Betnovate to affected areas. See our ketoconazole shampoo guide.
- After Cocois descaling — for thick psoriatic scale: use Cocois as directed, wash it out, dry the hair, then apply Betnovate so it can reach the skin. See our Cocois guide.
- Not with other steroid products — do not combine with Dovobet, Etrivex or other corticosteroids on the same areas unless your prescriber has specifically directed it; the steroid effects add up.
- Other hair products — wait until the liquid has fully dried before using styling products, and keep heated styling tools away until then.

Before and after: what to expect
Before treatment, an inflamed scalp typically shows red patches, scale and itching. As the steroid takes effect, itching and soreness often settle first, followed by less redness and scale. Once the scalp improves, the leaflet allows the number of applications to be reduced. Results vary from person to person, and scale that is very thick may need a descaling product first.
NICE recommends reviewing treatment 4 weeks after starting. If your scalp has not improved by then, or is getting worse, contact your prescriber — the diagnosis may need revisiting, or a different option such as Etrivex or Dovobet may be more appropriate.
UK safety and prescribing information
- Legal status: Betnovate Scalp Application is a prescription-only medicine (POM). It is prescribed only after a clinical assessment; our pharmacist independent prescribers may decline and refer you to your GP if it is not suitable.
- Licensed use: scalp psoriasis, seborrhoeic dermatitis of the scalp and inflammation associated with severe dandruff. Not for babies under 1 year; courses in children limited to 5 days.
- Common side effects: burning, pain, irritation or itching where it is applied (up to 1 in 10 people). Thinning skin, stretch marks, visible veins and hair loss are very rare.
- Course limits: NICE advises a 4-week break between courses of a potent steroid where possible, and no continuous use of a potent steroid at any site for longer than 8 weeks.
- Steroid withdrawal: the MHRA warns that stopping after long-term use can cause redness, stinging and burning that may spread beyond the treated area. If your skin gets worse within 2 weeks of stopping, do not restart treatment without speaking to your prescriber.
- Interactions: tell your prescriber if you take ritonavir or itraconazole, which can increase the amount of steroid in the body.
- Reporting side effects: you can report suspected side effects to the MHRA through the Yellow Card scheme website or the Yellow Card app.
