Finasteride is a prescription-only oral medication and the most widely prescribed treatment for male pattern baldness (androgenetic alopecia) in the UK. Sold under the brand name Propecia, it is taken as a 1mg tablet once daily. It belongs to a class of drugs called 5-alpha reductase inhibitors.
Unlike topical treatments such as minoxidil, finasteride works systemically — tackling the hormonal root cause of male pattern hair loss rather than simply stimulating follicles from the outside.
MHRA & FDA approved: Finasteride 1mg has regulatory approval from both the UK's MHRA and the US FDA for the treatment of male pattern baldness — making it one of only a handful of treatments with that dual credential. Generic finasteride is equally effective as branded Propecia and considerably less expensive.
How Finasteride Works
The primary driver of male pattern baldness is dihydrotestosterone (DHT) — a hormone derived from testosterone. In genetically susceptible men, DHT binds to hair follicle receptors and gradually miniaturises them, causing hairs to become progressively thinner and shorter until the follicle stops producing hair altogether.
Finasteride works by inhibiting the type II 5-alpha reductase enzyme, which is responsible for converting testosterone into DHT. By blocking this conversion, finasteride reduces DHT levels in the scalp by up to 70%. Lower DHT means follicles are no longer being attacked — they stabilise, and many begin producing thicker, healthier hair again.

Does Finasteride Work?
The clinical evidence is robust. Large-scale, placebo-controlled trials published in peer-reviewed journals have consistently demonstrated finasteride's effectiveness:
- 85% of men show improvement (hair maintenance or regrowth) after 5 years of continuous use
- Around 65% of users experience visible regrowth — not just a slowing of loss
- Men who took a placebo in the same trials continued to lose hair, confirming the drug's effect
- Results are most pronounced at the vertex (crown) but also significant at the hairline
Start early for best results. Finasteride is most effective when started before significant follicle miniaturisation has already occurred. If you have noticed early signs of hair thinning or recession, seeking treatment sooner rather than later gives you the best chance of maintaining and recovering your hair.
How Long Does Finasteride Take to Work?
| Timeframe | What to expect |
|---|---|
| 0–3 months | Initial phase — shedding may temporarily increase as the hair cycle resets. This is normal and not a sign of failure. |
| 3–6 months | Most men notice a reduction in hair shedding. The rate of loss slows noticeably. |
| 6–12 months | Stabilisation. Some men begin to see early signs of regrowth at the crown or hairline. |
| 12–24 months | Visible regrowth, where it occurs, typically becomes clearly apparent. Maximum results in most users. |
| 2+ years | Ongoing maintenance of results with continued daily use. |
Patience is essential. Many men abandon treatment before results become visible. Commit to at least 12 months before assessing effectiveness.
Side Effects of Finasteride
Finasteride is well tolerated by the vast majority of men. The most commonly discussed side effects relate to sexual function, occurring in approximately 1–2% of users at the 1mg dose:
- Decreased libido — reported in approximately 1–2% of users
- Erectile dysfunction — similarly uncommon at the 1mg dose
- Reduced ejaculate volume — rare at the hair loss dose
These side effects, when they occur, usually resolve on stopping treatment or over time. The 1mg dose used for hair loss is significantly lower than the 5mg dose used for prostate conditions, which reduces the risk profile considerably.
2025 EMA safety update: The European Medicines Agency confirmed that suicidal thoughts can occur as a very rare side effect of finasteride. If you experience mood changes or thoughts of self-harm while taking finasteride, stop immediately and contact your GP. In a medical emergency, call 999.
Not for women: Finasteride must not be handled or taken by women, especially those who are pregnant or may become pregnant. Always disclose your full medical history during your online consultation, including any existing conditions and medications.
Who Is Finasteride Suitable For?
Finasteride 1mg is suitable for adult men experiencing male pattern hair loss (androgenetic alopecia). It is not suitable for women. It is most effective in men with mild to moderate hair loss, particularly at the crown. It is generally less effective once hair loss has progressed to complete baldness at any given area.

Does finasteride work for hair loss?
Yes — finasteride is the most clinically effective oral treatment for male pattern baldness. Studies over 5 years show 85% of users experience improvement, with many seeing genuine regrowth. It works by blocking the enzyme that converts testosterone into DHT, the hormone responsible for follicle miniaturisation.
How long does finasteride take to work?
Most men notice reduced shedding within 3–6 months. Visible regrowth, where it occurs, typically becomes apparent after 12 months of consistent daily use. Commit to at least 12 months before judging results.
Do I need a prescription for finasteride in the UK?
Yes — finasteride is a prescription-only medicine. You can obtain a prescription via Access Doctor's GPhC-regulated online consultation, reviewed by GPhC-registered pharmacist independent prescribers, with next-day delivery available.
What happens if I stop taking finasteride?
If you stop finasteride, the protective DHT-blocking effect ends. Hair loss typically resumes within 6–12 months, returning to where it would have been without treatment. Finasteride must be taken continuously to maintain results.
Is finasteride safe to take long-term?
Yes — finasteride has been in clinical use for over 25 years and has a well-established long-term safety profile at the 1mg hair loss dose. It is MHRA and FDA approved. Always discuss your full medical history with your prescriber and report any mood changes immediately.
