Two things work against phenoxymethylpenicillin in a full stomach.
First, it is acid-labile — unstable in stomach acid. Some of each dose is destroyed before it can be absorbed, and the longer a tablet sits in an acidic stomach, the more is lost. Food slows stomach emptying considerably, so a tablet taken after a meal spends much longer in exactly the environment that degrades it.
Second, food physically interferes with absorption. The drug has to reach the wall of the small intestine to cross into the bloodstream. Mixed into a meal, less of it makes contact and less gets through.
The combined effect is a meaningfully lower peak concentration in the blood. For a single dose that hardly matters. Across twenty to forty doses in a five to ten day course, consistently taking them with food means running the whole treatment at a lower level than intended — which is how an infection that should have cleared ends up lingering.
This is also why nausea is more noticeable with penicillin V than with some other antibiotics. An empty stomach is harder on the gut. It is a real trade-off rather than an oversight, and the answer is water and regular meals between doses, not abandoning the timing.
Why amoxicillin is different
People often assume all penicillins share the same instructions, and get caught out. They do not.

A practical schedule for four doses a day
Four doses a day on an empty stomach sounds impossible until you build the doses around your meals rather than the other way round.

Adjust the clock times to your own routine. The doses do not need to be exactly six hours apart, and chasing precision at the cost of actually taking them is the wrong trade. Reasonably even spacing you can sustain for a week beats a perfect schedule you abandon on day two.
The one thing to avoid is grazing. If you snack continuously through the day, there is never a clear two-hour window. During the course, try to keep eating to defined meals so the gaps exist.
The twice-daily alternative
UK guidance allows adults 1000mg twice daily as an alternative to 500mg four times daily. If the four-times schedule looks unmanageable alongside the food requirement, this is worth raising with your prescriber.
Two doses a day is far easier to fit around meals: one on waking before breakfast, one at bedtime two hours after your evening meal. The empty-stomach rule still applies to both, but you only need two clear windows instead of four.

- Sit upright for a few minutes after taking it rather than lying down
- Give it time. Nausea often settles after the first day or two
