This is a prescription-only ear spray that combines three active ingredients: dexamethasone 0.1% w/w, a corticosteroid that reduces the inflammation and swelling behind much of the pain of an ear infection; neomycin sulfate 0.5% w/w, a broad-spectrum aminoglycoside antibiotic that works by stopping bacteria from producing the proteins they need to survive; and glacial acetic acid 2% w/w, which is antibacterial in its own right and helps restore the ear canal's natural acidic environment, making it a less hospitable place for bacteria to grow.
It is licensed for the treatment of outer ear infections, known medically as otitis externa and often called swimmer's ear. It is supplied as a 5ml bottle fitted with a metered spray device, so each press of the actuator delivers a measured dose directly into the ear canal.
If you have been prescribed Otomize Ear Spray in the past, this is what you will now receive. Otomize is no longer available under that brand name, and its manufacturer, Ennogen Healthcare International, has replaced it with this generic version. The active ingredients and their strengths are identical, so there is no change to the medicine itself, only to the name on the bottle.
This is a prescription-only medicine. A UK-registered prescriber will review your consultation answers to confirm that an ear spray of this kind is appropriate for your symptoms, and in particular that it is safe for you given the restrictions around a perforated eardrum and grommets.

Shake the bottle well before each use. If the spray is new, or if you have not used it for a week or more, prime it first by pointing the nozzle away from yourself and anyone else and pressing the actuator a few times until a fine spray appears.
The usual dose for adults and children aged 2 and over is one spray into the affected ear three times a day. Place the tip of the nozzle gently at the opening of the ear and press the actuator once. Keep using the spray for two days after your symptoms have cleared, rather than stopping as soon as you feel better, as this reduces the chance of the infection coming back.
This spray is for use in the ear canal only. Do not use it anywhere else on the body, do not inhale it, and do not swallow it. Keep using it exactly as your prescriber has directed, and speak to a clinician if there has been no improvement after about a week.

Ennogen Healthcare, the manufacturer, withdrew the Otomize brand name in mid-2025 and now supplies the same medicine as a generic product instead. It was a change to the branding, not to the medicine, so anyone who was previously prescribed Otomize will simply receive this version now.
No. This spray must not be used if you have a perforated eardrum, if a perforation is suspected, or if you have grommets fitted. Neomycin is ototoxic and can cause permanent hearing damage if it reaches the middle ear. If you are unsure whether your eardrum is intact, have your ear examined before using any ear preparation. A prescriber may be able to suggest an alternative, such as a ciprofloxacin-based drop, which is not affected by the same restriction.
The usual course is one spray into the affected ear three times a day, continued until two days after your symptoms have cleared. Courses are deliberately kept short, because excessive or prolonged use carries its own risk of hearing damage, particularly where the skin of the ear canal is broken or damaged. If your symptoms have not started to improve after about a week, speak to a clinician rather than continuing.
It can be used in children aged 2 and over, but not in infants or children under 2 years old. Care is particularly important in children, because the risk of hearing damage from excessive or prolonged neomycin use is a greater concern in this age group. A child should always be assessed by a clinician before an ear preparation is used.
