Tadalafil vs Sildenafil : Which is better?

Tadalafil vs Sildenafil: As two of the most well-known erectile dysfunction (ED) medications, sildenafil and tadalafil are commonly prescribed by GPs to men of all ages who are experiencing impotence. However, while they do both belong to the same class of drugs known as ‘phosphodiesterase type 5 (PDE5) inhibitors’, meaning they work in a similar way, there are a number of differences between sildenafil and tadalafil

In this article, we compare these phosphodiesterase-5 (PDE5) inhibitors to help you understand which medication may better suit your specific needs.

Tadalafil (Cialis) and sildenafil (Viagra) are the two most widely prescribed erectile dysfunction medications in the UK. Both belong to the same drug class (PDE5 inhibitors) and are highly effective — but there are important differences that make one or the other a better choice depending on your lifestyle, preferences, and health profile. This guide compares them in detail to help you and your prescriber make the right choice.

How Both Medications Work

Both sildenafil and tadalafil are PDE5 inhibitors. They block the phosphodiesterase type 5 enzyme in penile smooth muscle, preventing the breakdown of cyclic GMP (cGMP). The resulting accumulation of cGMP allows blood vessels in the penis to relax more easily when sexually stimulated, enabling increased blood flow and facilitating an erection. Sexual stimulation is required with both medications.

When Sildenafil Is the Better Choice

  • You prefer a shorter-acting medication with a defined window of effectiveness
  • Cost is a consideration — sildenafil generics are widely available at lower prices
  • You prefer to take medication only immediately before planned sexual activity
  • You are already accustomed to sildenafil and it is working well for you

When Tadalafil Is the Better Choice

  • You want more spontaneity — not planning sex around medication timing
  • You have sex frequently (more than twice a week) — daily 5mg tadalafil may be more convenient and cost-effective
  • You prefer to take medication with food without worrying about absorption effects
  • Your partner prefers the flexibility that a 36-hour window provides (studies show higher partner preference for tadalafil)

Important: Do not take sildenafil and tadalafil together or on the same day. Both are PDE5 inhibitors — combining them provides no additional benefit and significantly increases the risk of side effects including dangerously low blood pressure.

Side Effects — Similarities and Differences

Both share similar common side effects (headache, flushing, nasal congestion, indigestion). Notable differences:

  • Sildenafil is more commonly associated with temporary visual disturbances (blue tinge, altered colour discrimination)
  • Tadalafil is more commonly associated with back pain and muscle aches, related to its longer systemic presence

Frequently Asked Questions

What is the main difference between tadalafil and sildenafil?

The key difference is duration: sildenafil (Viagra) lasts 4–6 hours and should be taken 1 hour before sex. Tadalafil (Cialis) remains active for up to 36 hours, making it the ‘weekend pill’ and offering significantly more spontaneity. Tadalafil also has the advantage of not being affected by food.

Which is more effective — tadalafil or sildenafil?

Clinical trials show similar efficacy: tadalafil worked in 81% of users, sildenafil in 82%. There is no clinically significant difference in effectiveness. However, tadalafil’s longer duration means partner preference studies often favour tadalafil due to the flexibility it provides.

Can I take tadalafil every day?

Yes. Tadalafil 5mg is available as a once-daily formulation for continuous ED management — removing the need to plan around medication timing entirely. This is a popular option for men who are sexually active more than twice a week.

Does food affect tadalafil or sildenafil?

Food significantly affects sildenafil — taking it after a large, high-fat meal reduces its absorption and effectiveness. Tadalafil is not meaningfully affected by food, making it more flexible for use around mealtimes.

Can you take tadalafil and sildenafil on the same day?

No. Taking both within the same 24-hour period risks serious side effects including severe hypotension, nausea, dizziness, and prolonged painful erection (priapism). Use one or the other, not both.

Which ED medication is better for spontaneous sex?

Tadalafil is generally considered better for spontaneity due to its 36-hour duration of action. Once taken, it provides a window of readiness that accommodates more natural sexual timing. Sildenafil requires more planning due to its 1-hour onset and 4–6-hour window.
