When to get urgent help: Call 999 or go to A&E if you have signs of a blood clot or stroke while taking norethisterone: pain, swelling or redness in one leg; sudden chest pain or breathlessness; coughing up blood; sudden weakness or numbness on one side of the body, difficulty speaking or a sudden severe headache. Also call 999 for a severe allergic reaction, such as swelling of the face, lips, tongue or throat, wheezing or difficulty breathing. Stop taking it and see a doctor the same day if your skin or eyes turn yellow, you get a migraine for the first time, or you have sudden changes to your vision.
Key fact: norethisterone 5mg tablets, one three times a day starting at least 3 days before your period is due, usually delay it while you keep taking them, for the course length your prescriber advises. Courses are usually shorter than a full month, so covering all of Ramadan may need a different option. Start your consultation at least a week ahead.

Use your last 2 to 3 cycles. If your cycle varies, use the earliest likely date.

Covering Ramadan
Ramadan lasts 29 or 30 days. If your period is due early in the month, a norethisterone course may need to run for most of it, which is longer than the 10 to 14 days in the Primolut N licence. Tell your prescriber the dates you want to cover so they can advise. Options include:
- Covering part of the month, such as the last ten nights, with a shorter norethisterone course.
- A longer course, if your prescriber agrees it is appropriate (NHS guidance allows up to 3 to 4 weeks if necessary).
- Running a combined pill back to back, if you already take one. See delaying your period on the pill.
Taking norethisterone while fasting
The tablets can be taken with or without food. During a fast, take all three doses in the hours you are not fasting and spread them as evenly as you can, for example at iftar, before you sleep and at suhoor. In the UK summer, when fasting hours are long, the gaps will be shorter than usual; keep the same times each day. If you can’t fit three doses in, ask your prescriber for advice before you start.
Whether taking a tablet affects your fast, and whether to delay your period at all, are religious questions. Scholars and schools differ, so seek guidance from a religious authority you trust.
Planning for Hajj or Umrah
Pilgrimage means long flights, heat, crowds and long days on your feet. A few practical points:
- Long flights: norethisterone and long periods of sitting both slightly raise the risk of blood clots. Tell your prescriber about your flight, move around during it and drink plenty of water.
- Heat: store the tablets below 25°C in their original packaging, out of direct sun. Keep them in your hand luggage.
- Pack enough: take enough tablets to cover delays to your journey home.
- Stay hydrated: dehydration in the heat is a risk in its own right, and it also matters for circulation.

Exams
Start 3 days before your period is due and continue through your exams. You can stop after your last exam; your period usually starts within 3 days. Some people get spotting while taking the tablets; keep taking them, and contact your prescriber if the bleeding becomes heavy. If side effects such as headaches would affect your revision, try a course before your exam period if you have the chance, and talk to your prescriber.
Who should not take it
Norethisterone is suitable for most healthy women after a prescriber check, but it carries a small risk of blood clots and isn’t suitable for everyone. You should not take it if you are pregnant or have had a blood clot, heart attack, angina, stroke, liver disease, porphyria, hormone-sensitive cancer, migraine with aura or unexplained vaginal bleeding. UK safety and prescribing information
- Legal status: norethisterone is a prescription-only medicine (POM). At Access Doctor it is prescribed only after an online consultation with a GPhC-registered pharmacist independent prescriber, who may decline and suggest an alternative or that you see your GP.
- Licence: it is the only progestogen licensed in the UK to delay periods (NHS Specialist Pharmacy Service). The dose is 5mg three times a day, starting 3 days before your period is due.
- Course length: take it for as long as your prescriber advises. The Primolut N licence describes courses of no longer than 10 to 14 days, and NHS Specialist Pharmacy Service guidance says it can be taken for up to 3 to 4 weeks if necessary.
- Not a contraceptive: use condoms or another method while you take it.
- Blood clots: part of each dose is converted in the body to ethinylestradiol, the oestrogen in the combined pill (about 4–6 micrograms for every 1mg, according to the Primolut N product information), so the same blood clot warnings apply. Your prescriber will check risk factors such as a previous clot, smoking, a BMI over 30, being over 35, recent surgery and long-haul travel.
- Pregnancy and breastfeeding: do not take it if you are or might be pregnant. The Primolut N product information says it should be avoided while breastfeeding, so ask your prescriber.
- Reporting side effects: you can report suspected side effects to the MHRA through the Yellow Card scheme website or the Yellow Card app.
