What is Kliofem?

Kliofem Tablets are a type of hormone replacement therapy (HRT) or menopause tablets that are prescribed to relieve the symptoms of menopause in women.

Kilofem is sometimes used as a second-line postmenapusal treatment, it is beneficial in preventing osteoporosis in postmenopausal women.
Kliofem Tablets are a continuous form of HRT they contain a form of the normal female hormones oestrogen and progestogen called estradiol (oestradiol) and norethisterone.

Kilofem HRT is only to be used by post-menopausal women (after the menopause).

They should not be used by men or children. HRT is only for females.

Kilofem is commonly misspelled as Kliofem.

How does Kliofem work?

During the time of menopause, the naturally produced levels of oestrogen in a women’s body decreases. This produces the symptoms of menopause such as hot flushes, night sweats, mood swings, vaginal dryness and irregular periods.

Kliofem Tablets contain estradiol (oestradiol), a naturally occurring form of oestrogen, to top up falling levels of the hormone and relieve the symptoms of menopause.

The HRT tablets also contain norethisterone, a synthetic form of progesterone.

Hormone replacement tablets

Estradiol (oestrogen) stimulates the growth of the womb lining, unfortunately, this h has been linked to endometrial cancer, therefore it is essential to take it with norethisterone (progestogen) which opposes this effect.

Kliofem Tablets do not produce a monthly bleed, they are most suitable for women whose periods have completely stopped after menopause.

How to use Kliofem

Using Kilofem tablets

  • 28-day easy to use a calendar pack
  • One tablet should be taken every day.
  • There are no breaks
  • Try to take at the same time every day
  • The tablet should be swallowed whole ideally with a glass of water

Missed dose

If it’s less than 12 hours since you missed the dose, take the missed dose.

If it is more than 12 hours, you cannot make up for it, just take the next days dose at the regular time.

What dosages are available for Kliofem Tablets?

Each tablet contains 2mg estradiol (as estradiol hemihydrate) and 1mg norethisterone acetate.

These tablets come in a 28-day calendar pack.

Kliofem Tablets could increase the levels of certain medicines in the blood, you should tell your doctor if you're taking any of these:

  • Ropinirole
  • Aminophylline
  • Selegiline
  • Tizanidine

Do not take these tablets if you are pregnant or breastfeeding.

Does Kliofem increase risk of cervical cancer?

Does HRT increase the risk of breast cancer?

All HRT options are thought to increase the risk of breast cancer. The risk is higher if using a combined HRT.

Vaginal oestrogens are not associated with increased risk of breast cancer.

HRT use for 1 year or less, is associated with the lowest risk of breast cancer.

Does HRT increase the risk of ovarian cancer?

Both the oestrogen only HRT and the combined HRT are thought to increase the risk of ovarian cancer. Interestingly, the risks go back to baseline when HRT is stopped.

Does HRT increase the risk of womb (endometrial) cancer?

Risks associated with endometrial cancer from HRT depend on the type of HRT being used.

Oestrogen-only HRT is associated with endometrial cancer and the risk increases the longer it is taken. Oestrogen only HRT is only offered to women who have had a previous hysterectomy.

Combined HRT will reduce the risk of endometrial cancer and should be offered to women with intact uterus. It should be noted that the combined HRT options increase the risk of breast cancer. It is very important to discuss the treatment options with your doctor first before starting.

HRT’s effect on heart and circulation

Oestrogen contributes to heart health by its beneficial effects on cholesterol lowering. Oestrogen also contributes to improved blood circulation by its actions on blood vessel dilatation. Oestrogen also has been shown to absorb free radicals which can damage arteries.

Studies have shown that supplemental oestrogen (menopause hormone replacement)

or similar hormones may help improve cardiovascular health and improve overall survival.

After accounting for age, coronary calcium score and cardiovascular risk factors including diabetes, high blood pressure, and high cholesterol, women receiving 0estrogen were:

  • 30% less likely to die than those not on hormone therapies
  • 20% more likely to have a coronary calcium score of zero
  • 36% less likely to have a coronary calcium score above 399, indicating severe atherosclerosis and high risk of heart attack

Tell your doctor about any other medicines you are taking, including over-the-counter medicines, as these could affect how Kliofem Tablets work or increase your risk of side effects.

It is important to tell your doctor or pharmacist what medicines you are already taking, including those bought without a prescription and herbal medicines, before you start treatment with this medicine. Similarly, check with your doctor or pharmacist before taking any new medicines while taking this one, to make sure that the combination is safe.

The following medicines may potentially reduce the blood level and effect of this medicine, which could cause irregular menstrual bleeding or your symptoms to come back:

  • aprepitant
  • bosentan
  • barbiturates
  • carbamazepine
  • cobicistat
  • crizotinib
  • dabrafenib
  • efavirenz
  • eslicarbazepine
  • fosaprepitant
  • fosphenytoin
  • modafinil
  • nevirapine
  • oxcarbazepine
  • phenobarbital
  • phenytoin
  • primidone
  • rifabutin
  • rifampicin
  • ritonavir
  • rufinamide
  • telaprevir
  • the herbal remedy St John's wort (Hypericum perforatum)
  • topiramate

Some women with diabetes may need small adjustments in their dose of insulin or antidiabetic tablets while taking this medicine. You should monitor your blood sugar and seek advice from your doctor or pharmacist if your blood sugar control seems to be altered after starting this medicine.

This medicine may oppose the effect of medicines used to lower high blood pressure. Your blood pressure will usually be checked periodically while you are taking HRT, but this is particularly important if you are also taking medicines for high blood pressure.

This medicine may also oppose the fluid-losing effect of diuretic medicines.

This medicine may decrease the amount of the antiepileptic medicine lamotrigine in the blood. As this could increase the risk of seizures coming back or getting worse, the medicine may not be recommended for women who take lamotrigine on its own for epilepsy.

Kliofem in Pregnancy and breastfeeding

Kliofem Tablets are not a contraceptive and will not prevent pregnancy.

Side effects that have been commonly reported include:

  • Mood swings
  • Headache
  • Nausea and vomiting
  • Abdominal pain, bloating, flatulence, indigestion
  • Menstrual bleeding or spotting
  • Vaginal thrush
  • Increase in the size of uterine fibroids
  • Breast pain, tenderness or enlargement
  • Fluid retention, causing swelling (oedema).
  • Vaginal bleeding
  • Sudden chest pain
  • Leg cramps
  • Blood clots
  • Severe headaches
  • Visual disturbances

You should not take these tablets if you are allergic to any of their ingredients.

Kliofem and depression

Although the exact mechanisms underlying the antidepressant effects of oestrogens remain unclear, there is increasing evidence indicating that they may act via both serotonergic (5HT) and noradrenergic pathways. Monoamines (e.g., serotonin) are known to be inhibited by oestrogens.

Preliminary evidence suggests that both hormonal replacement therapy (HRT; e.g., oestrogen plus progestin) and antidepressant therapies (ADTs; e.g., selective serotonin reuptake inhibitors [SSRIs]) share common mechanisms of action. Moreover, there is preliminary evidence suggesting that HRT alleviates the full spectrum of postmenopausal mood disorders. Thus, many perimenopausal and postmenopausal women experiencing depression or a major depressive episode might benefit from treatment with an SSRI alone or in combination with HRT.
