Medical disclaimer: This article is for informational purposes only. Dermovate is a prescription-only medicine classified as a very potent corticosteroid. It must only be used as directed by a qualified prescriber. Our prescribers are GPhC-registered pharmacist independent prescribers.
Dermovate is one of the most powerful topical corticosteroids available in the UK. Its active ingredient, clobetasol propionate 0.05%, is reserved for severe or treatment-resistant inflammatory skin conditions where milder corticosteroids have been insufficient. Available as cream, ointment, and scalp lotion, Dermovate provides rapid and potent anti-inflammatory relief — but must be used carefully, for the shortest time necessary, following a prescriber’s instructions.

Dermovate is indicated for short-term treatment of severe, inflammatory, or resistant skin conditions, including:
- Severe or resistant eczema (atopic, contact, or discoid)
- Psoriasis (plaque type) — where moderate steroids have not been effective
- Lichen planus — a chronic inflammatory condition causing purple, itchy patches
- Discoid lupus erythematosus (DLE)
- Severe reactions to insect stings or bites
- Other severe dermatoses where rapid symptom control is needed
Important: Dermovate is not appropriate for acne, rosacea, skin infections, perioral dermatitis, or routine use on the face, groin, or eyelids. Always follow prescriber guidance on which areas to treat.
How Dermovate Works
Clobetasol propionate suppresses the overactive immune response that drives inflammatory skin conditions. It works by entering skin cells and binding to glucocorticoid receptors, which suppresses the production of pro-inflammatory cytokines, prostaglandins, and other mediators responsible for redness, swelling, and itching. It also has vasoconstrictive properties that reduce the blood vessel dilation associated with skin inflammation.
Dermovate Formulations: Which Is Right for You?
| Formulation | Best For | Key Points |
|---|---|---|
| Cream | Moist or weeping skin (eczema, inflamed lesions) | Apply thinly 1–2x daily; do not use on infected skin |
| Ointment | Dry, thick, scaly skin (psoriasis, chronic eczema) | Occlusive — do not use under tight bandages unless prescribed |
| Scalp Lotion | Scalp psoriasis and seborrheic dermatitis | Apply 1–2x daily; flammable — avoid flames; max 50ml/week |
How to Use Dermovate Safely
1
Wash hands: Before and after each application.
2
Apply thinly: Use the minimum amount needed to cover the affected area — a thin smear is sufficient.
3
Do not exceed 50g/week: Across all Dermovate applications combined.
4
Short courses only: Generally no more than 4 weeks continuously. If no improvement within 1 week, contact your prescriber.
5
Missed dose: Apply as soon as you remember. If nearly time for next dose, skip the missed one — do not double up.
6
Storage: Store at room temperature, away from sunlight and heat. Keep out of reach of children.
Side Effects and Safety
Because clobetasol propionate is a very potent corticosteroid, it carries more significant risks than milder topical steroids, particularly with prolonged or excessive use:
- Skin thinning (atrophy) — particularly on areas of thin skin; use minimum effective amount
- Stretch marks (striae) — with prolonged use on flexible skin areas
- Perioral dermatitis / steroid rosacea — if applied to face; avoid facial use
- Adrenal suppression — rare but possible with large amounts over extensive areas; monitor for chronic fatigue, weight loss
- Increased susceptibility to skin infections — avoid use on infected skin
- Visual disturbances — rare; report blurred vision immediately
Red flag symptoms requiring urgent review: Stop Dermovate and contact your prescriber immediately if you develop signs of skin infection (spreading redness, warmth, pus), visual changes, or symptoms suggesting adrenal suppression (unexplained fatigue, weight loss, dizziness).

What is Dermovate used for?
Dermovate (clobetasol propionate 0.05%) is a very potent topical corticosteroid used to treat severe, resistant, or short-term inflammatory skin conditions including severe eczema, psoriasis, discoid lupus erythematosus, and lichen planus. It is only recommended when milder steroids have failed.
How strong is Dermovate?
Dermovate is classified as a ‘very potent’ (Class IV) topical corticosteroid — the strongest class available. It is approximately eight times more potent than hydrocortisone 1% and significantly more potent than mild-to-moderate steroids like Eumovate or Betnovate.
How often should Dermovate be applied?
Dermovate cream, ointment, or scalp lotion should be applied thinly once or twice daily to affected areas only. Treatment should generally not exceed 4 weeks. Do not exceed 50g (or 50ml) per week across all applications.
Who should not use Dermovate?
Dermovate is not suitable for children under 18, people with skin infections (bacterial, fungal or viral), rosacea, acne, perioral dermatitis, or those allergic to clobetasol propionate. It should not be applied to the face, eyelids, genitals, or skin folds without specialist advice.
Can I use Dermovate on my scalp?
Dermovate scalp lotion is specifically formulated for the scalp. Apply 1–2 times daily to affected scalp areas. Keep away from open flames as the lotion is flammable. Do not use more than 50ml per week.
