Dermovate Scalp Application is a prescription-only liquid that treats serious scalp problems like psoriasis (those thickened red patches with silvery scales) and other stubborn skin conditions that haven't got better with milder steroid creams, ointments or lotions. It contains a very strong steroid called clobetasol propionate 0.05% – it's one of the most powerful topical steroids you can get. Because it's so strong, it's only ever used for short courses and only when other treatments have failed. In medical terms, clobetasol propionate belongs to the most potent class of topical corticosteroids (Group IV). It works by calming down the overactive immune response in your scalp – reducing the redness, swelling, flaking and itching that make scalp psoriasis so uncomfortable. Importantly, Dermovate is not a shampoo – it's a leave-on liquid that you apply directly to your scalp and leave there. You don't lather it up or rinse it out immediately.
Dermovate contains clobetasol propionate 0.05% – the same active ingredient as the Dermovate creams and ointments used on the body. In the UK steroid grading system, it sits right at the top of the four-rung ladder, above potent steroids like betamethasone (Betnovate) and mometasone (Elocon). To give you an idea of just how strong it is: clobetasol is approximately 600 times more potent than mild hydrocortisone 1%. That's why it works so quickly on thick, stubborn scalp psoriasis – but it's also why you need to be very careful with it. When you apply it, the steroid penetrates into the scalp skin cells and stops them from producing the inflammatory chemicals that cause redness, swelling and itching. Because it works by changing how your cells behave, it's not instant – you'll usually need a few days of use before you see the full effect. But the results can be dramatic when it works.
Using Dermovate correctly makes all the difference. Here is the step-by-step method from the UK patient information leaflet: 1. Prepare your scalp – Your hair and scalp should be clean and completely dry before you start. 2. Wash your hands – Before you touch the bottle, wash your hands thoroughly. 3. Part your hair – Use your fingers or a comb to part your hair and expose a small area of affected scalp. 4. Apply the liquid – Unscrew the bottle cap. Place the nozzle directly on the scalp area that needs treating. Gently squeeze the bottle to apply a thin, even layer of liquid. Don't pour large amounts on – a little goes a long way. 5. Rub in gently – Use your fingertips to gently massage the liquid into your scalp. 6. Repeat for other areas – Move to the next patch of affected scalp and repeat the process until all problem areas are covered. 7. Let it dry – The liquid dries quickly on its own. Do not use a hairdryer – the product is flammable and heat can cause serious reactions. 8. Wash your hands again – After you've finished applying, wash your hands thoroughly to avoid accidentally spreading the steroid to your face or eyes. 9. You keep this up until you see a clear improvement in your scalp – this might take a few days or up to a couple of weeks. Once your scalp is looking and feeling better, your doctor will probably tell you to reduce the frequency. You might drop to once a day, or even just a few times a week, to keep the condition under control. The goal is always to use the lowest amount possible for the shortest time needed. The absolute maximum for a single course is 4 weeks – after that, if you still need treatment, your doctor should switch you to a milder steroid. Why is there a 4-week limit? Clobetasol is extremely strong. The official UK prescribing information states very clearly that clobetasol belongs to the most potent class of topical corticosteroids and that "prolonged use may result in serious undesirable effects". If you need topical steroid treatment for longer than 4 weeks, your doctor is supposed to move you to a less potent preparation. This isn't just a suggestion – it's a safety measure to protect your scalp from permanent thinning and to prevent your body from absorbing enough steroid to affect your natural hormone system.

However, the official guidance emphasises that children are much more likely than adults to develop both local side effects (like skin thinning) and systemic side effects (where the steroid affects their whole body). For children: - Courses should be limited to just a few days if possible. - Treatment should be reviewed weekly. - Only the absolute minimum amount needed should be used. - Long-term continuous use should be avoided entirely. If you're considering Dermovate for a child, it must be prescribed by a doctor who has carefully weighed the risks and benefits. Do not use it on a child under 1 year of age – that's a formal contraindication.
The official Summary of Product Characteristics says there is inadequate evidence of safety in human pregnancy, and animal studies have shown a very small risk of developmental abnormalities. Pregnancy: Dermovate is a very potent steroid, and as a general rule, doctors prefer to use the mildest effective steroid during pregnancy. However, the amount of steroid absorbed from the scalp is generally low. Some doctors may decide that a short course (1-2 weeks) on a limited area is acceptable if your scalp psoriasis is severe and other treatments have failed. But you should never start Dermovate during pregnancy without a full discussion with your doctor. Breastfeeding: If you use Dermovate while breastfeeding, make sure the liquid is completely dry before any skin-to-skin contact with your baby. When used exactly as prescribed – short course, small area – serious side effects are rare. But because Dermovate is so strong, side effects can happen if you use too much or use it for too long. Common (mild, often settle quickly): - Mild stinging or burning when you first apply it – this usually goes away after a few uses. - Dryness or itching at the application site. Less common but more significant (usually from overuse): - Thinning of the scalp skin (atrophy) – the skin becomes shiny, fragile and tears easily. This can be permanent. - Telangiectasia – fine red lines (spider veins) appearing on the scalp. - Stretch marks on the scalp. - Acne or small bumps on the scalp or forehead. - Changes in skin colour – the treated area may become lighter or darker. Topical steroid withdrawal (TSW) – also called red skin syndrome – is a recognised reaction where, after prolonged use of a potent or very potent steroid, stopping the treatment causes a severe rebound. Do not use Dermovate Scalp Application on: - Any infection on your scalp – bacterial (like impetigo), fungal (like ringworm), or viral (like cold sores or shingles). - Children under 1 year of age – this is a firm contraindication. - Any area other than your scalp – the skin on your face, armpits, groin and body is much thinner and more prone to side effects. Dermovate is designed specifically for the scalp. - Broken, damaged or thin skin on the scalp – it can absorb too much and cause stinging or irritation. - If you are allergic (hypersensitive) to clobetasol propionate or any of the other ingredients listed on the bottle. Also, if you have psoriasis, your doctor will want to see you more often. There is a rare risk that using strong steroids on psoriasis can trigger a serious form called pustular psoriasis. Dermovate itself does not directly cause permanent hair loss. However, severe scalp psoriasis often comes with temporary hair shedding. The thick, inflamed plaques can grip onto several hair follicles, and when the scale loosens and comes away, clumps of hair can come with it. This shedding is from the inflammation and the physical grip of the psoriasis plaques, not the steroid itself. Once the psoriasis is treated and the scalp calms down, hair typically grows back normally. If you are noticing significant or patchy hair loss, speak to your doctor – there may be another cause that needs investigation.
This is one of the most common questions – and the answer requires care. The official guidance says that repeated but short courses of clobetasol propionate may be used to control flare-ups. That means you can use it again when your scalp psoriasis flares, provided you follow the same rules each time: a short course (usually 2-4 weeks), twice-daily application until it settles, then stop. However, if you find you need to use Dermovate more than 2-3 times a year, or if it stops working as well as it used to, speak to your doctor. You may need a different maintenance approach – for example, a milder steroid (like Betacap) for ongoing control, or a vitamin D analogue (like Silkis), or a combined approach. Also, there is a recognised phenomenon called tachyphylaxis – the skin can become less responsive to a steroid over time, meaning you need more of it to get the same effect. If that happens with Dermovate, your only option is to switch to a different class of treatment entirely.
If you have used it for a standard 2-4 week course, you can stop without any special steps – just stop. Your skin may still be improving for a few days after you stop, which is normal. If you have used it for months continuously (for example, you kept reusing it without medical supervision), stopping suddenly can cause a rebound flare. The psoriasis can come back worse than before – sometimes turning into a more unstable form called pustular psoriasis. If you have been using it for months, do not stop suddenly. Speak to your doctor. They will help you taper off – for example, reducing from twice daily to once daily for a week, then to every other day for a week, then to twice a week, before stopping.
Storage: - Store below 25°C in a dry place. - Protect from light – keep the bottle in its outer carton between uses. - Keep out of the sight and reach of children. - Do not freeze or store in a hot bathroom cabinet. Shelf life: The shelf life is 3 years from the date of manufacture. Once opened, the product does not have a specific "use within" period, but as a good rule, if you haven't used it for several months, check that it still looks clear and does not have any particles or an unusual smell before using it again. Disposal: Do not throw Dermovate away in household waste or pour it down the sink or toilet.
