Tadalafil in the UK: How to Get It Safely Online
Medical disclaimer: This article is for informational purposes only and does not constitute medical advice. Tadalafil is a prescription-only medicine. Always consult a qualified prescriber before starting or changing any medication. In a medical emergency, call 999 or go to your nearest A&E.
Tadalafil is the UK's second most prescribed treatment for erectile dysfunction and the active ingredient in Cialis. Its 36-hour duration, independence from food, and availability as a once-daily low dose make it the most distinctive and versatile ED medication available. This guide covers how tadalafil works, how to get it in the UK, dosage, side effects, and contraindications. For a head-to-head comparison with sildenafil, see: Tadalafil vs Sildenafil. For a comparison of all four MHRA-approved ED medications, see: Viagra vs Cialis vs Levitra vs Stendra.

What Is Tadalafil?
Tadalafil is an MHRA-approved phosphodiesterase type 5 (PDE5) inhibitor, the same class of drug as sildenafil (Viagra), vardenafil (Levitra), and avanafil (Spedra). Tadalafil was originally licensed under the brand name Cialis, manufactured by Lilly. Since the patent expired, generic tadalafil has been widely available in the UK at significantly lower cost.
Unlike sildenafil, which lasts 4–6 hours and is affected by food, tadalafil lasts up to 36 hours and is completely unaffected by food or alcohol timing. This longer window gives it a very different profile of use — and makes it the preferred choice for men who want a more spontaneous sexual experience.
Tadalafil is also the only ED medication with a dual licence: it is approved for both erectile dysfunction and benign prostatic hyperplasia (BPH), making it the medication of choice for men with both conditions.
Tadalafil & Revatio
Tadalafil at a much lower dose (2.5mg and 5mg) is also licensed for pulmonary arterial hypertension under the brand name Adcirca. This is an entirely separate indication managed by specialist physicians and should not be confused with ED treatment doses (10mg and 20mg).
How Does Tadalafil Work?
During sexual stimulation, nerves in the penis release nitric oxide, which triggers production of cyclic GMP (cGMP). cGMP relaxes the smooth muscle lining penile arteries, allowing blood to flow in and produce an erection. The enzyme PDE5 normally breaks cGMP down — tadalafil blocks PDE5, allowing cGMP to persist longer and making erections significantly easier to achieve.
Sexual stimulation is still required. Tadalafil does not cause an automatic erection — it enhances the body's natural erectile response when arousal is present.
Tadalafil Dosage in the UK
Tadalafil for ED is available in two on-demand doses and one daily dose. The choice depends on your lifestyle, frequency of sexual activity, and tolerability.
| Formulation | Dose | Timing | Duration | Notes |
|---|---|---|---|---|
| On-demand (standard) | 10mg | 30 min before sex | Up to 36 hours | Recommended starting dose for most men |
| On-demand (increased) | 20mg | 30 min before sex | Up to 36 hours | If 10mg is insufficient; maximum on-demand dose |
| Daily | 5mg | Same time each day | Continuous | For men who want spontaneous sex or have sex ≥2x/week. For on-demand vs daily comparison, see: Tadalafil vs Sildenafil |
Never take more than one on-demand dose in a 24-hour period. Older men or those with kidney or liver impairment should start at 5mg on-demand and increase only under prescriber guidance.
Unlike sildenafil, tadalafil does not need to be taken on an empty stomach — it can be taken with or without food, and the timing of a meal has no effect on absorption.
How to Get Tadalafil in the UK
Tadalafil is a prescription-only medicine (POM) in all doses in the UK. Unlike sildenafil 50mg (Viagra Connect), there is no over-the-counter tadalafil available — a prescriber consultation is required for all doses.
| Route | Product | Doses Available | How to Access |
|---|---|---|---|
| NHS prescription | Generic tadalafil | 5mg, 10mg, 20mg | GP or NHS online service. Subject to NHS prescribing criteria. |
| Private prescription (online) | Generic tadalafil | 5mg, 10mg, 20mg | GPhC-registered online pharmacy such as Access Doctor |
| Branded private prescription | Cialis (Lilly) | 5mg, 10mg, 20mg | GP or private clinic. Clinically equivalent to generic; significantly more expensive. |
NHS Tadalafil: What You Need to Know
NHS prescribing of tadalafil follows the same criteria as sildenafil: it is available on the NHS for men with ED associated with diabetes, multiple sclerosis, Parkinson's disease, prostate cancer, renal failure requiring dialysis, spinal cord injury, or severe pelvic injury, and where ED causes severe distress. Men who do not meet these criteria may be directed to private purchase.
Getting Tadalafil Online from a Registered Pharmacy
Access Doctor's GPhC-registered pharmacist independent prescribers can assess your suitability for tadalafil via a confidential online consultation — no GP appointment required. The process takes around 5 minutes. If appropriate, a prescription is issued and your medication is dispatched in plain, unmarked packaging, typically arriving next working day.
Always verify that an online pharmacy is registered with the GPhC at pharmacyregulation.org before purchasing any prescription medication. See: Why Kamagra Is Dangerous for the risks of unregulated online suppliers.
Side Effects of Tadalafil
Tadalafil's side effect profile overlaps significantly with sildenafil, since both are PDE5 inhibitors. However, its longer half-life means side effects, when they occur, can persist for longer — though they remain mild for most men.
Common (may affect up to 1 in 10 people)
- Headache — the most frequently reported side effect; caused by cerebral vasodilation
- Facial flushing — warmth and redness, usually mild and transient
- Nasal congestion or rhinorrhoea — temporary; generally less frequent than with sildenafil
- Indigestion (dyspepsia) — particularly if taken close to a meal
- Back pain and muscle aches (myalgia) — more commonly reported with tadalafil than other PDE5 inhibitors, due to its long systemic presence. Usually resolves within 48 hours.
- Dizziness — blood-pressure-lowering effect; avoid standing quickly
Uncommon
- Visual disturbances — less common with tadalafil than sildenafil due to greater PDE5 selectivity
- Palpitations or tachycardia
- Hypotension, particularly in combination with antihypertensives or alpha-blockers
Seek Emergency Care Immediately If You Experience
An erection lasting more than 4 hours (priapism — attend A&E immediately), sudden vision loss in one or both eyes, sudden hearing loss, chest pain, or collapse. These reactions are rare but require urgent treatment. Stop tadalafil and call 999.
Who Should Not Take Tadalafil
A clinical assessment before prescribing is specifically designed to identify the following. Do not take tadalafil if you:
- Take nitrate medicines in any form — GTN spray or patches, isosorbide mononitrate or dinitrate, or recreational “poppers” (amyl nitrite, butyl nitrite). The combination with tadalafil causes a severe and potentially fatal drop in blood pressure. This is an absolute contraindication with no exceptions.
- Take riociguat (Adempas), used for pulmonary hypertension
- Have been advised to avoid sexual activity for cardiovascular reasons
- Have had a stroke or myocardial infarction within the past 6 months
- Have severe hepatic impairment
- Have severe hypotension (resting blood pressure below 90/50 mmHg)
- Have uncontrolled hypertension (above 170/110 mmHg)
- Have a hereditary degenerative retinal disorder such as retinitis pigmentosa
Key Drug Interactions
| Drug/Class | Interaction | Management |
|---|---|---|
| Nitrates (all forms) | Severe hypotension — potentially fatal | Absolute contraindication. Do not combine. |
| Riociguat | Severe hypotension | Contraindicated |
| Alpha-blockers (tamsulosin, doxazosin) | Additive blood-pressure-lowering | Start at 5mg on-demand; allow 4+ hours between doses |
| CYP3A4 inhibitors (clarithromycin, ketoconazole, itraconazole) | Increase tadalafil plasma levels | Do not exceed 10mg; monitor for adverse effects |
| Ritonavir & HIV protease inhibitors | Markedly increase tadalafil levels | Maximum 10mg every 72 hours |
| Antihypertensives | Additive blood-pressure-lowering | Generally manageable; inform prescriber |
| Alcohol | Additive vasodilation; increases dizziness and flushing | Limit to 1–2 units |

Frequently Asked Questions
What is the difference between tadalafil and Cialis?
There is no pharmacological difference. Cialis is the original branded product manufactured by Lilly. Generic tadalafil — available in the UK since the patent expired — contains the same active ingredient at the same doses and must demonstrate MHRA bioequivalence. Both work identically. Generic tadalafil is typically significantly cheaper.
How long does tadalafil last?
Up to 36 hours — significantly longer than sildenafil (4–6 hours) or vardenafil (4–6 hours). This means you can take tadalafil on-demand and remain responsive to sexual stimulation for a full day and a half, rather than planning around a shorter window.
Can I take tadalafil with food?
Yes — unlike sildenafil, tadalafil is not affected by food or the timing of meals. You can take it with or without food and at any time relative to eating.
What is the starting dose of tadalafil for erectile dysfunction?
The recommended starting dose for most men is 10mg, taken approximately 30 minutes before anticipated sexual activity. If 10mg is well tolerated but insufficient, it can be increased to 20mg. If 10mg is effective but causes side effects, it can be reduced to 5mg. Never take more than one on-demand dose in 24 hours.
Does tadalafil cause back pain?
Back pain and muscle aches (myalgia) are more commonly reported with tadalafil than with other PDE5 inhibitors, due to its longer presence in the body. It usually affects the lower back and resolves within 24–48 hours. If it is persistent or severe, discuss switching to a different ED medication with your prescriber.
Is tadalafil available on the NHS?
Yes, subject to criteria. NHS guidelines permit tadalafil prescribing for men with ED associated with specific conditions (diabetes, prostate cancer, multiple sclerosis, and others) or where ED causes severe distress. Men who do not meet these criteria can access treatment through a GPhC-registered online pharmacy like Access Doctor.
