Nizoral is a medicated shampoo that treats and prevents dandruff and seborrhoeic dermatitis – the medical name for a common skin condition that causes a red, scaly, itchy scalp. It contains an active ingredient called ketoconazole (1% for the over-the-counter version), which is an antifungal medicine. Unlike a standard shampoo that just washes away flakes, Nizoral tackles the underlying cause of dandruff. The key to its effectiveness is that it works for days after you use it, not just while you're in the shower.
Most dandruff is caused by an overgrowth of a yeast called Malassezia that lives naturally on everyone's scalp. Ketoconazole works by disrupting the cell membranes of this yeast, stopping it from growing and reproducing. This reduces the flaking, itching, and irritation. Crucially, ketoconazole continues to bind to the upper layers of your scalp's skin cells for several days after you rinse it off. Even when the shampoo is gone, a small amount of the active ingredient remains in the skin. That's why you don't need to use it every day – twice a week is enough to keep the yeast under control. The antifungal effect builds up over time, which is why you need to use it for 2‑4 weeks to clear the problem properly, not just one wash.
There are two phases to using Nizoral: a treatment phase and a maintenance phase. Don't skip the second one – that's what stops the dandruff coming back. Treatment phase (getting rid of the dandruff): Wet your hair and scalp thoroughly. Apply enough Nizoral to make a good lather. Gently massage the lather into your scalp, not just the hair. Leave it on for 3 to 5 minutes before rinsing. This is important – the shampoo needs time to work. Many people rinse too quickly and then wonder why it didn't work. Use it twice a week (e.g., every Wednesday and Sunday) for 2 to 4 weeks. Maintenance phase (keeping the dandruff away): After the 2‑4 week treatment course, reduce your use to once every 1 to 2 weeks. This regular top‑up use prevents the yeast from growing back out of control. If you stop completely, the dandruff will likely return within a few months. For some people with particularly stubborn seborrhoeic dermatitis, a doctor may recommend a stronger 2% version or a different schedule – but for standard over‑the‑counter use, the above is the standard UK regimen.
A 60ml bottle is usually enough for the full 4‑week treatment phase (8 washes) and several months of maintenance use. You only need about a 20p‑sized amount for short hair – slightly more for long or thick hair. Because the shampoo lathers well, a little goes a long way. Most people find one bottle lasts them 3‑4 months when you factor in the reduced maintenance schedule.
Most people notice a significant reduction in flaking and itching within 2 weeks (after about 4 washes). The scaling and redness typically clear noticeably by the end of the 4‑week treatment period. If you have seen no improvement after 4 weeks of using it twice a week, you should stop and see your doctor or pharmacist. Possible reasons it might not work include: you may have a different scalp condition (such as psoriasis or eczema that needs a different treatment), you may have a steroid‑resistant fungal strain (uncommon with ketoconazole), or you may need the prescription‑strength 2% version rather than the 1% over‑the‑counter version.
Ketoconazole shampoo is absorbed through the skin in only very small amounts, so the risk to a developing baby or a nursing infant is considered very low. However, as a precaution, UK guidelines advise avoiding prolonged or excessive use during pregnancy and breastfeeding. Using it for the standard 2‑4 week treatment course as directed on the pack is unlikely to cause any problems, but you should speak to your doctor or pharmacist before use if you are pregnant, trying to conceive, or breastfeeding, just to be safe.
Most people use Nizoral without any problems, but side effects can occur in a small number of users. The most common ones are relatively mild: Mild scalp irritation, burning sensation or itching – this usually settles after the first few uses. If it continues or gets worse, stop and speak to a pharmacist. Dry or oily changes in the hair or scalp. Some people find their hair feels slightly different when using medicated shampoos. A change in hair texture – some users report their hair feeling more brittle or straw‑like. This usually resolves when you switch to maintenance mode or stop using the shampoo. Temporary hair loss – this has been reported but is rare. The hair loss is usually mild and reverses when you stop using the shampoo. If you notice significant shedding, stop and see your doctor. Rare allergic reactions – signs include a skin rash, hives, or worsening itching. If you develop a spreading rash or swelling of the face, lips or tongue, stop immediately and seek medical attention. Discolouration of chemically treated or grey hair – this is very rare but has been reported. If you have bleached, permed or colour‑treated hair, you may want to test the shampoo on a small hidden section first. If you experience any side effects that worry you, stop using the shampoo and speak to a doctor or pharmacist.
Yes, but with caution. Nizoral is generally safe on colour‑treated hair, but in very rare cases it can cause discolouration, particularly on grey hair or hair that has been chemically treated (bleached, permed or relaxed). If you have chemically treated hair, you may want to: Do a patch test on a small, hidden section of hair first. Limit the contact time – leave the shampoo on for 3 minutes rather than 5. Use a good conditioner afterwards, as ketoconazole can be slightly drying. Most people with colour‑treated hair use Nizoral without any issues, but if you notice any colour change, stop using it and switch to a non‑medicated dandruff shampoo.
Do not use Nizoral shampoo on: Broken or inflamed skin on the scalp – if you have open sores, cuts, or a very raw, weeping scalp, the shampoo could cause stinging and irritation. If you are allergic to ketoconazole or any of the other ingredients in the shampoo. Ingredients are listed on the pack – common ones include sodium lauryl sulfate, cocamide MEA and perfume. Ringworm or athlete's foot – these are caused by different types of fungus that ketoconazole shampoo is not designed to treat. You need a dedicated antifungal cream for those. Lice or nits – Nizoral does not kill head lice. You need a specific lice treatment like Derbac M. Also note that Nizoral is for external use only. Do not swallow it. If swallowed by accident, contact a doctor or poison control centre immediately. Avoid getting it in your eyes – if you do, rinse thoroughly with cool water.
Wash every third day
